  • Cancellation Policy

    Life can be hectic and scheduling conflicts come up. We do require 24 business hours notice to cancel or reschedule an appointment. If an appointment is canceled less than 24 business hours in advance they will incur a charge of 50% of that appointment fee. For example if the patient needs to cancel a Monday appointment it needs to be done on Friday. If an appointment is cancelled with less than 4 hours notice on the same day the patient with incur a charge of 100% of the appointment cost. If the patient does not show or call to cancel, the full amount may also be charged. After 2 “no shows” or 3 cancellations in a row, we may remove you from our patient caseload in order to prioritize other patients. We thank you for respecting both our time as well as those patients on our waitlist.

  • Payment Policy

    Payment is due in full at time of service. You may wish to provide a credit card to be kept on file in which case the card will be charged following service. Payments can also be taken In the form of HSA/FSA, cash or check.

  • Privacy

    All documentation and forms are HIPAA compliant and will not be shared with anyone without your written consent.

  • Chaperone Policy

    You have the right to a chaperone or second person in the room during evaluation and/or treatment. The chaperone can be a family member or trusted friend, we do not provide a chaperone. You may also waive your right to a chaperone.
