What Is Birth Prep, Exactly?

Birth prep is a series of one-on-one pelvic floor physical therapy sessions designed to prepare your body (specifically your pelvic floor, core, hips, and spine) for the demands of labor and delivery. It's not a childbirth education class. It's not just about breathing. It's hands-on, personalized, and rooted in what your body actually needs going into birth.

Whether you're planning a vaginal birth, a C-section, a VBAC, or a home birth, your body benefits from intentional preparation. Labor is a physical event, and like any physical event, the more prepared your body is, the better it tends to go.

What We Work On

Every person's body is different, so your sessions will be tailored to you: your history, your birth plan, and where you are in your pregnancy. But here's what birth prep at Lotus typically looks like.

Pelvic floor balance: strength and relaxation

This is the piece most people don't expect. When people think pelvic floor work, they think Kegels. But for birth, the ability to let go matters just as much as strength. A pelvic floor that can't fully relax may resist rather than work with your baby during delivery. We assess both and address whatever your body needs most.

Learning how to push effectively

Pushing is a skill, and most people have never been taught how to do it. We work on pushing mechanics in multiple labor positions so that when the time comes, you're not guessing. Knowing how to coordinate your breath, your core, and your pelvic floor during pushing can genuinely impact how long pushing takes and your risk of tearing.

Hip, pelvis, and spine mobility

Your pelvis needs to be able to move freely during labor. We work on joint mobility and soft tissue flexibility so your body can open and shift the way it needs to as your baby descends.

Reducing tension that can contribute to tearing

Perineal tissue that's tight or restricted is more prone to tearing during delivery. Through manual therapy and tissue preparation, we work to improve elasticity and reduce unnecessary tension in the weeks before your due date.

Supporting optimal baby positioning

How your baby is positioned going into labor matters more than most people realize. We incorporate techniques to support ideal fetal positioning, which can contribute to a smoother, more efficient labor.

Managing pregnancy discomfort

Birth prep sessions often do double duty as prenatal care. Pelvic pressure, round ligament pain, low back and SI joint pain, hip discomfort, pubic symphysis dysfunction: these are all things we address. They're common in the third trimester, but they don't have to just be something you push through.

Breathing and pressure management

We teach 360 breathing (how to coordinate your diaphragm and pelvic floor together) which is foundational for labor and carries over into your postpartum recovery as well.

When Should You Start?

We typically recommend starting around 32 to 36 weeks, though earlier is always welcome if you're also dealing with pain, leaking, or other pelvic floor symptoms during pregnancy. Many of our patients start pelvic floor care in the first or second trimester and naturally shift into birth-specific prep as they get closer.

Already in your third trimester and haven't started yet? Don't stress. Even one or two sessions can make a real difference.

Bring Your Partner

Birth is a team effort, and we mean that literally. We offer partner sessions and genuinely encourage partners to come in. Whether that's a spouse, a doula, a family member, or whoever will be in the room with you, having them present during a session means they understand what you're working on and can actually support you during labor. We'll walk them through positioning, comfort measures, how to help you breathe and push effectively, and what to expect from your body in the moment. Partners who feel prepared are so much more helpful in the room, and it takes a lot of pressure off you.

What Comes After?

Birth prep is only part of the picture. Postpartum recovery is where we pick the work back up: pelvic floor rehab, healing from tearing or C-section, diastasis recti, core rebuilding, and a gradual return to movement and exercise. Patients who come in for birth prep tend to move through postpartum recovery more smoothly, because their body was prepared going in rather than starting from scratch.
