Office Policies

  • We understand that schedules can change and unexpected conflicts may arise. If you need to cancel or reschedule your appointment, we kindly ask for at least 24 business hours’ notice.

    Appointments canceled with less than 24 business hours’ notice will incur a fee of 50% of the scheduled appointment cost. For example, if you need to cancel a Monday appointment, we must receive notice by Friday.

    Appointments canceled with less than 4 hours’ notice may be charged 100% of the appointment fee. Patients who do not arrive for their scheduled appointment and do not notify the office may also be charged the full appointment fee.

    Because appointment times are reserved specifically for you, repeated missed appointments impact other patients waiting for care. After two no-shows or three consecutive late cancellations, we may need to remove you from our active patient schedule in order to accommodate other patients.

    We appreciate your understanding and your help in respecting both our time and the time of others on our waitlist.

  • Payment is due at the time of service.

    For convenience, you may choose to keep a credit card on file, which will be charged following your appointment. We also accept the following forms of payment:

    • Credit or debit card

    • HSA/FSA cards

    • Cash

    • Check

  • We are committed to protecting your privacy. All documentation and patient forms are HIPAA-compliant, and your personal health information will not be shared without your written consent, except as required by law.

  • Patients have the right to request a chaperone or second person present during any evaluation or treatment.

    You may bring a family member or trusted support person to serve as your chaperone if you wish. Please note that our office does not provide chaperones.

    Patients also have the option to waive their right to a chaperone if they prefer.
